MEET OUR TEAM
Mary Costantino, MD, FSIR
Mary Costantino is a highly distinguished Interventional Radiologist, having been awarded some of the most prestigious accolades available in her specialty. Medical School at the University of California at Los Angeles (UCLA) and Internship at Stanford University Department of Internal Medicine prepared her for a Diagnostic Radiology Residency at Oregon Health and Science University (OHSU), followed by an Interventional Radiology Fellowship at Georgetown University Hospital.
National Leader & Mentor
As Medical Director for Advanced Vascular Centers, Dr. Costantino leads a team of exceptional medical professionals who share her vision of providing an exceptional patient experience, and the most advanced care available to the community.
Board Certified in Diagnostic Radiology with a Certificate of Additional Qualification (CAQ) in Interventional Radiology; Highest level of certification offered in her specialty. Recognized as a Society of Interventional Radiology (SIR) Fellow (FSIR), which is one of the most prestigious honors granted to SIR members and the ultimate recognition of professional
achievement.
Active in SIR, she is a voting member of the SIR executive committee, and Division Chair of the Private Practice section,
representing approximately 85% of all SIR membership.
A recognized thought leader and innovator in the field, Dr. Costantino is widely published, and frequently invited to speak at local, national, and international professional conferences and vendor-sponsored events.
She actively mentors other Interventional Radiologists in early phases of their career, and is especially proud of mentoring
women in the field, and working to close the gender gap in the specialty.
Community Activist
Since 2006, Dr. Costantino has been practicing primarily in Oregon, where she is actively making a difference in the lives of her patients. With a deep compassion for her community and for her patients, she is a passionate advocate for informed consent. Dr. Costantino is committed to providing every patient the opportunity to truly understand their condition, treatment options, and care plan. She is acutely aware of the disparity of care in minority populations,
and often speaks and advocates on behalf of marginalized patients to ensure that they have appropriate access to care.
Volunteer & Mother
In her free time, Dr. Costantino is active in many philanthropic and professional organizations, including membership in the Philanthropic Educational Organization (PEO). She has served on the Board of Directors, and is now Treasurer, of Rose Haven; a day shelter for women and children in downtown Portland. She is also raising her two teenage sons in the greater Portland area.
Andrew T. Gentile, MD
Dr. Gentile is a board-certified vascular surgeon whose practice now focuses entirely on the treatment of venous disease, including venous insufficiency, varicose veins, DVT and venous stasis ulcers.
Education
Dr. Gentile was raised in Southern California and graduated with honors with degrees in both Biochemistry and Biology from California Polytechnic State University in San Luis Obispo, California. He attended Creighton University Medical School in Omaha, Nebraska where he graduated Magna Cum Laude. Dr. Gentile completed his internship and residency in General Surgery at Oregon Health and Science University (OHSU) in Portland, Oregon. He then completed a 2-year fellowship in Vascular Surgery at the University of Arizona in Tucson, Arizona.
History
After completing his specialty vascular training, he practiced Vascular Surgery in Tucson for five years and then moved back to Portland and joined Northwest Kaiser Permanente, practicing primarily at the Kaiser Sunnyside Hospital. He was Surgeon and Medical Director with Inovia Vein in Happy Valley. He has developed an expertise in comprehensive wound care and in treating complicated patients with deep venous obstruction and blood clotting in the legs.
Carrie Hayes DMSc, MHS, PA-C
Carrie Hayes is a highly experienced Physician Associate (PA) and Diagnostic Medical Sonographer with over 20 years in the radiology and healthcare fields. She has built a dynamic career in both clinical and academic settings, excelling in Interventional Radiology (IR) and ultrasound education.
Professional Leadership & Advocacy
Carrie is a passionate advocate for advanced practice providers (APPs) in Interventional Radiology (IR). Over the past decade, she has supported IR residents, fellows, and APPS through leadership roles in the Society for Interventional Radiology (SIR), including six years on the NP/PA Section Governing Council as Clinical Practice Chair, Chair, and now Past Chair. She has led initiatives like APP onboarding content for the SIR Annual Conference and collaborated with organizations like the American College of Radiology. Currently, she is working with the NCCPA to develop an IR Certificate of Added Qualification (CAQ) for APPs.
Global Ultrasound Education
As a former Director of Ultrasound for RAD-AID International, Carrie has advanced ultrasound education in over 30 countries, including Haiti, Tanzania, Nepal, and Ghana. She has developed sustainable programs, and currently partners with organizations including the Pan American Health Organization (PAHO), Inteleos™ and the Global Ultrasound Institute™, through supporting traditional and hybrid certification pathways to expand access to quality ultrasound in underserved regions.
Education & Mentorship
Carrie actively mentors and educates future healthcare leaders . She has been a preceptor for aspiring physician associates in the Houston Medical Center and Stanford Health Care and now mentors aspiring professionals in Central Oregon and abroad , supporting local institutions through the Oregon Medical Education Foundation and nationally through the Society of Interventional Radiology.
Personal Interests
Carrie lives in Southern Oregon, where she enjoys hiking, cooking, road trips, and renovating a mountain home on 40 acres in the Siskiyou National Forest. She shares her home with her partner Matthew, two French bulldogs (Fox and Yoda), and a rescue cat named Bones.
